I am grateful for almost 5-months completely sober from Narcotics and Alcohol for the first time in decades. My last detox (detox #22 total) I was blessed with the opportunity to start Vivitrol injections 
It has finally been a game changer for me. Minimal cravings at most. Vivitrol is very expensive ($1635 per shot + Dr visit) but Medicaid covers it! I am 6-months in, and haven't touched Heroin or Alcohol. God bless
